NOAC Medical Form Due Date Fast Approaching

All NOAC Medical Forms are due by July 10, 2015. Only the BSA Annual Health and Medical Record form encoded with your personal barcode will be accepted.

In order to attend the conference, each guest and staff member must have a physical exam any time after July 31, 2014. Guests and staff must submit an electronic medical form with a personalized barcode. No other medical form will be accepted. Your medical must have the guests or staff members personalized barcode.

Submission Process

The step by step submission form process is available online. Each guest and each staff member must have their own MyScouting account on https://myscouting.scouting.org/ prior to starting the process.
